The ACT Peer Support Specialist provides direct treatment, rehabilitative, and supportive services in a community setting to members. Works as a member of a clinical team to assess member needs and preferences, participates in the development of individual service plans, acts as a member advocate, and liaises with families and community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage. Serves as the team’s lead member advocate and mentor. Is a self-disclosed mental health member and is willing to provide a member’s perspective on the team’s service provision.
MINIMUM QUALIFICATIONS:
High School Diploma or GED
Peer Certification required upon hire or within 30 days of starting
AZ-approved Peer Employment Training Program and current certification (as of 2013)
Basic knowledge and understanding of mental illness
Ability to obtain and maintain a Level One Fingerprint Clearance Card and any other required background checks
Ability to meet Copa’s authorized driving requirements, including valid AZ Driver’s License upon hire, 39-month MVR, proof of insurance, and vehicle registration (if applicable)
JOB SUMMARY (including, but not limited to):
Meets with members in the community in accordance with established policies, procedures, and clinical protocols to provide supportive counseling, skill development, and other support services as needed
Works with the member’s Prescribing Clinician, Nurse, and other members of the clinical team to ensure timely development and implementation of the individual’s service plan (including all necessary assessments) and crisis plan
Coordinates with all parties involved in a member’s treatment according to policy and clinical protocols, including attending inter- and intra-agency staffings, communication via phone and letters, and facilitating service/support delivery as indicated in the member’s service plan
Assists members and families in accessing needed services and obtaining entitlements, including identifying funding sources
Responsible for transporting members to necessary services, as outlined in their service plan
Assumes “primary contact” responsibility for a percentage of team members to:
Maintain accurate, thorough, and up-to-date documentation in the medical record (per policy, procedure, and clinical protocol)
Keep weekly member schedules updated
Act as the first contact for those members and their families
