Job Description:

The Nebraska Chronic Disease Prevention & Control Program is seeking a motivated and self-driven professional to provide innovation, leadership, and guidance to organizations and partners in Nebraska striving to prevent cardiovascular disease and manage associated risk factors.

This position serves as the subject matter expert for cardiovascular disease prevention and management programming with a focus on healthcare system interventions (i.e. electronic medical record utilization, team-based care, and self-measured blood pressure monitoring) and community-clinical linkages (i.e. linking patients to community programs/resources and lifestyle change programs).

This position can be located in either Lincoln or Omaha.


 

Job duties include:

  • Serve as the subject matter expert for cardiovascular disease prevention and management;
  • Assist in development of and leading the implementation of grant activities to ensure the goals and objectives of the program are met; assist in the design, implementation, and monitoring of program evaluation and performance measures to ensure program goals and objectives are met;
  • Conduct quality improvement processes as needed to improve program effectiveness and/or efficiency;
  • Develop annual grant applications, budgets, work plans, and progress reports; develop, implement, and monitor contracts/subawards to assure adherence to federal and state regulations;
  • Engage internal/external partners and facilitate collaborative relationships; plan and provide technical assistance and training to contractors/subrecipients, healthcare clinics/organizations, local health departments and other partners on evidence-based strategies, policies, and system changes;
  • Plan and facilitate meetings for small and large groups.


 

Requirements / Qualifications:

Minimum Qualifications: Bachelor’s degree or equivalent coursework/training in community health education, biological science, adult/continuing education or related field AND experience in public/community health education activities with responsibility for: developing, evaluating, and leading health education program activities; assessing health needs; writing and designing educational and informational material, training aids, and presentations; preparing grant proposals; or planning and leading group sessions. Related experience may substitute for a Bachelor’s degree on a year-for-year basis.

Preferred Qualifications: Master's degree in public or community health, healthcare, or related field. Minimum of 1 years' experience  working in public or community health, healthcare, or related field.

Other: Valid Driver's License is required.

Knowledge, Skills, and Abilities:

Knowledge of the principles and practices of public health planning and implementation; the principles of grant writing, reporting, and grant administration; the practices and techniques of developing partnerships and collaborations.

Ability to maintain accurate and thorough records, design educational and informational materials, training aids, and presentations; prepare grant proposals; plan and lead group sessions; interpret and apply federal grant requirements, agency rules, policies and procedures; communicate effectively; prioritize and manage time; problem solve and make decisions; balance needs, requirements, and work load for multiple projects; self-motivate and work independently with minimal supervision.
